Access: No appointments necessaryโ”simply walk in or call for transportation.

Purpose: To provide a โMedical Homeโ: with universal access to health care for the medically underserved population in Sussex County, Delaware.

Goal: to move Sussex County toward 100% health care access & 0% disparity.

Who Can Participate: the Uninsured, the homeless, migrant Farm Workers, Public Housing Residents, At-risk School Children, etc.

Medical Services: Primary and preventive care (all ages), dental care, mental health and substance abuse services, pharmacy, X-ray and diagnostic laboratory services, case management, referrals for specialty and emergency care, etc.

Enabling Services: Transportation, Language and culturally competent services, health education, assistance to access social services, etc.

Cost of Services: Small nominal fee based on sliding scale and ability to pay.

System of Care: TTF Community Health Center provides universal access to health care through in-house resources and collaborative agreements with DHSS agencies, and private health care providers. The network is coordinated by TTF.

Patient Rights: Respect, Confidentiality, Safety, Information sharing with enrollees, Consent, Privacy, Appropriate assessment, Choice, No One is denied care, etc.

Partners: Bureau of Primary Health Care (BPHC), Delaware Health Care Commission, Delaware Health and Social Services, all local hospitals, Managed Care Organizations, Delaware Medical Society, TTF Network Providers.

Governance: A 15-member Board composed of professionals for strategic and tactical management; A 20 member steering committee reflective of groups to be served for community and operational management (All volunteers).

Through the efforts of Dr. Tonwe, his associates, the Board of Directors, the Steering Committee and many volunteer hours, The Takpoma Foundation is having the Grand Opening of the Community Health Center in Millsboro, Delaware. The hours will be 10 AM until 3 PM on Wednesday, July 31, 2002. The Millsboro Chamber of Commerce and honored guest will host the ribbon cutting at 12 noon at 560 West DuPont Highway. Millsboro, Delaware.
In addition to a tour of the medical facility, a dental van will be on site for touring. The public is invited to visit the Community Health Center.
